The AcuSafe Risk Data Center was created to promote an awareness of
accidents involving chemical hazards, and educate the public and
industrial operators on the root causes of these incidents to reduce the
chance of such occurrences in the future. It contains information
about incidents in the past (Major
Historic Incidents, HMIRS)
and present (Monthly Capsule
Summary, News Archive), and refers
the reader to other major databases for
more information when the information is not available on the web site
directly. To understand the underlying causes of these incidents, there is
statistical information, a
section on lessons learned
from unsafe work practices, and a collection of publicly available accident
investigation reports.
AcuSafe strives to maintain an accurate and
complete collection of information on this subject. If you see an error or
