API releases 2nd Edition of Security Vulnerability Assessment Methodology
The American Petroleum Institute has released a 2nd edition of the Security Vulnerability Assessment Methodology.  AcuTech Consulting Group was a key contributor to developing the first edition and completing this latest edition which includes pipeline and transportation security vulnerability issues.  Copies are available by contacting Bill Erny at  Ernyw@api.org

3rd Edition of Frank P. Lees  “Loss Prevention in the Process Industries” Published

Over the last three decades the process industries have grown very rapidly, with corresponding increases in the quantities of hazardous materials in process, storage or transport. Plants are becoming larger and are often situated in or close to densely populated areas. Increased hazard of loss of life or property is continually highlighted with incidents such as Flixborough, Bhopal, Chernobyl and Piper Alpha.  Increasing attention must therefore be paid to the control of these hazards, and in particular the need to develop a comprehensive approach to the avoidance of human and economic loss; this is loss prevention.

Sam Mannan of the Mary Kay O’Connor Process Safety Center spearheaded this new edition of F.P Lees “Loss Prevention in the Process Industries”.  This classic reference work on loss prevention was rewritten, bringing every aspect up to date. New chapters are included on computers, artificial intelligence and expert systems, and new appendices feature recent disasters such as Bhopal and Piper Alpha.  Mr. David Moore, AcuTech Consulting Group, contributed the chapter on Process Security.  The many thousands of references have been updated along with standards and codes of practice. US 9/11 Report Released
The “Final Report of the National Commission on Terrorist Attacks Upon the United States” was recently released.  The executive and full reports are posted on the AcuSafe.com web site for public review.  The report provides a narrative of the terrorist attacks and recommendations to prevent or mitigate future attacks.   View the Executive Summary and Full Report.

Dr. Daniel Horowitz Appointed as Director of CSB Congressional, Public, and Board Affairs

Washington, DC, February 22, 2005 – Daniel Horowitz has been appointed Director of Congressional, Public, and Board Affairs for the U.S. Chemical Safety and Hazard Investigation Board (CSB), the agency announced today.  Dr. Horowitz, a special assistant to the board since joining the agency in 2000, will head the CSB’s congressional relations efforts, including contact with Members and staff in relevant appropriations and authorizing committees. As head of public affairs, he will continue to direct the CSB’s public outreach, including oversight of the agency’s public meetings, liaison between CSB Board Members, investigators and community officials, and the news media.

 

Chemical Safety Board Investigation Finds CTA Acoustics Explosion Caused by Dust Hazard That Was Known to Management, Phenolic Resin Supplier

Washington, DC,  February 15, 2005 - By a vote of 3-0, the U.S. Chemical Safety and Hazard Investigation Board (CSB) tonight approved the final report on the phenolic resin dust explosion at CTA Acoustics. In approving the staff report, the Board added a root cause related to Borden Chemical's product stewardship program (Borden Chemical supplied phenolic resin powder to CTA). The report identified four other root causes related to CTA's safety systems and the design of the CTA facility by former owner CertainTeed. The full report, as amended, will be available soon from CSB.gov, along with a transcript and videotape of the public meeting.

 

New Investigation:  Acetylene Service Company Gas Explosion, Perth Amboy, NJ  January 25, 2005
On January 25, 2005, a gas explosion killed three workers at the Acetylene Service Company plant in Perth Amboy, NJ. The blast originated in a wooden shed located near six large storage tanks that received liquid waste from the plant's acetylene generating system. The plant produces, repackages, and distributes acetylene used in welding.  CSB has initiated a full investigation.

 

Marcus Oil and Chemical Tank Explosion, Houston, TX, December 3, 2004

On the evening of December 3, a storage tank failed catastrophically at the Marcus Oil and Chemical polyethylene wax facility in Houston. The blast, which was felt up to 20 miles from the plant site, ignited large fires that burned for several hours, and two firefighters were injured during the emergency response. Off-site buildings near the facility – including two churches, a house, and a social club – exhibited structural damage, such as broken windows and cracked walls.  CSB has initiated a full investigation.

New EPA Web Site Reports Inspection and Enforcement
EPA’s new web page Enforcement and Compliance History On-line (ECHO), www.epa.gov/echo, is now available.  The site provides a search engine to determine whether compliance inspections have been conducted by EPA/state or local government, if violations were detected, and if enforcement actions were taken and penalties were assessed in response to environmental laws.

We collect stories about major spills and releases from around the globe. This month's installment (http://www.acusafe.com/Newsletter/Stories/0205News-MonthlyIncidents.htm) summarizes incidents brought to our attention between November 16, 2004 and February 15, 2004. Most of the incidents covered have caused significant injury, deaths, or major property damage.

1.  AcuTech’s Dave Moore to Speak at Global Security Asia (GSA) in Singapore
Mr. Dave Moore, CEO and President of AcuTech Consulting Group, will present a paper on the ‘Risk Analysis Methodology for Critical Asset Protection (RAMCAP)’ project at the Global Security Asia conference to be held in Singapore March 29 to 31, 2005.  Mr. Moore is the petroleum, chemical, and LNG industry sector coordinator for the RAMCAP project for the US Department of Homeland Security.  This national effort is developing uniform vulnerability assessment guidance.

2.  AcuTech to Present Security Vulnerability Assessment Overview Course in Singapore Following GSA Conference

Following the GSA conference in Singapore, Mr. Moore will teach a one day course on April 1.  The course will present the leading methodologies for analyzing security vulnerabilities, concepts of vulnerability assessment and security management of critical infrastructure, and means to mitigate undesired consequences by identifying and implementing security countermeasures.  Information on the program is available at www.acutech-consulting.com. 

3.  CCPS 20th International Conference to be Held April 10 to 13, 2005

The annual CCPS conference will focus on  Risk Management:  The Path Forward.  The conference is being held in conjunction with the annual AIChE meeting, along with the Loss Prevention and Plant Process safety meetings at the Hyatt in Atlanta Georgia.  AcuTech will present two papers during the conference, and will be available in Booth 207 in the CCPS exhibit area.  For more information on the conference go to http://www.aiche.org/ccps/icw/ 

4.  Special Government Focused SVA Class to be Held at API Headquarters in April

API and AcuTech have announced an added April date to the schedule of SVA workshops.  The April 26-27 session, to be held at API headquarters in Washington, DC, will focus on the API Security Vulnerability Assessment (SVA) methodology.   A special rate for government/NGO status has been established for this workshop.  For more information please view the SVA Washington DC Flyer and contact Janet Colbert at AcuTech Consulting Group.

2.  Martin Rose Joins AcuTech’s Eastern Business Group Staff
AcuTech Consulting Group is pleased  to announce the addition of Marty Rose, Senior Engineer, to AcuTech. Marty has a BSChE from

3.  David Heller Leaves CSB; Joins AcuTech Consulting Group
AcuTech Consulting Group is pleased to announce the addition of David Heller, Senior Principal Engineer, to AcuTech’s staff. David has a BSChE from Rensselaer Polytechnic Institute and joins us after 5 years as an investigation manager and investigator at the Chemical Safety Board (
CSB) in Washington DC. Prior to the CSB, David spent 13 years in various process safety management positions with Cytec Industries. He also worked in general process and process safety engineering positions for Air Products and Union Carbide.  David joined AcuTech January 4th and he will be establishing AcuTech's Washington, DC office.  David can be reached in Washington, DC at AcuTech’s new office number, 202-785-5161 or via email at dheller@acutech-consulting.com.
