EPA Issues Annual TRI Report for 1999
On April 11, 2001, EPA issued its annual Toxic Release
Inventory (TRI) report, which covers calendar year 1999. For a
complete copy of the report, fact sheets and information by zip

Overall, total on- and
off-site releases increased from 1998 to 1999 by 5 percent. The
original industries, coal mining and petroleum terminals and
bulk storage facilities reported decreases for that period. The
so-called "original industries" (mostly manufacturing
facilities) reported a 2.5% decrease, coal mining reported a 10%
decrease, and petroleum terminals and bulk storage facilities
showed a 5.5% decrease.

Flawed Management Supervision to Blame for 1999
Tosco Refinery Fire that Killed Four Workers
The CSB shared its conclusions about the 1999 tragedy at the
Avon Refinery, which was owned by Tosco Corporation at the time
of the incident. Two root causes of the tragedy were
identified: First, Avon refinery management did not have
an effective process for assessing the dangers of maintenance
operations and implementing needed safeguards. Second, neither
the parent Tosco Corporation nor the facility management had
investigated or corrected a pattern of serious unsafe practices
at the Avon refinery.” A

CSB's findings in more detail. An

CSB Issues Budget Justification Report
The U.S. Chemical Safety and Hazard Investigation Board (CSB)
has released its annual budget justification and performance
plan for fiscal year (FY) 2002. The new budget, transmitted to
Congress and the President, requests an appropriation of $9
million to support Board operations during the fiscal year
beginning October 1. Congress appropriated $7.5 million for CSB
operations in FY2001. Just over $1 million of the requested
increase is slated for personnel costs, due largely to the CSB’s
successful hiring program over the last twelve months. The
balance of the increase will fund additional training, contract
support, and office space rental. By law, the Board prepares and
submits its budget to Congress independent of the Executive
Branch.

Monthly Summary of Incidents, February 26, 2001 - April
12, 2001
We collect stories about
major spills and releases from around the globe. This month's installment
summarizes a total of 14 incidents brought to our attention between
February 26, 2001 and April 12, 2001. Most of the incidents covered have caused significant injury, deaths, or major property damage.
Accidents in Denmark, Spain, Canada, Brazil, Angola, Japan,
Turkey, and
the U.S. are represented. We welcome input to this effort by submitting summaries of other notable events to

Sinking of Petrobras' Offshore Oil Platform, the World's Largest
A series of explosions rocked a Petrobras Offshore Oil Platform on March
15; the one-year-oldstructure sank five days later. A total of
11 workers were killed and almost half a million gallons of oil
spilled into the ocean. For a chronology of the event,
speculation as to the cause of the tragedy, and a chronology of
recent Petrobras accidents, please read our
